Home Care Services
Nursing Visits
Registered Nurses assess patient's medical condition and work with the physician and other professionals to develop the plan of care which includes changing dressings, giving injections, instructing patient in self-care, teaching family members and monitoring progress. Skilled nursing care is delivered by a registered nurse or a licensed vocational nurse (with RN supervision). They are equipped with to provide nursing care in the home. Nurses are carefully screened and licensure is verified as a condition of employment. Nurses’ skills and experiences are considered when making patient assignment. Our effort is placed toward selecting a good match for you and your family’s needs.

Physical Therapy
Our Physical Therapists in coordination with the Physician develop and manage the physical therapy treatment plan to help patient regain flexibility, coordination and strength.

Occupational Therapy
The Occupational Therapists help patients to restore and improve their ability to perform task in living and working environments. Their goals are to help patients have independent and satisfying lives.

Speech Therapy
The Speech Therapists, through a rehabilitation program, help patients to manage conditions that affect speech, hearing, language, eating, swallowing and memory problems.

Medical Social Services
Our Medical Social Workers conduct a comprehensive assessment of the patient, their family, and the current environment to identify, address, and correct phyco-social stressors that have a negative effect on the patient. The Medical Social Worker is a support team member who is licensed by the state. He/she assist patient with referral/access to community resources, living and long–term financial arrangements, and long range planning. The Medical Social Worker may also provide short-term counseling.

Home Health Aide Care
Our Home Health Aides help patients with personal care needs such as feeding, repositioning or grooming following the instructions of the registered nurse and therapist. Home health aide services are delivered under the supervision of a registered nurse. For most programs, the aide is supervised by the RN at least every 14 days. Care delivered by the aide typically might include bathing the patient, changing linens, shampooing hair, and assistance with other activities of daily living. Aides are employed, carefully screened, and have experience/training in providing care in the home.
